The day of your appointment
It is a requirement that on your day of appointment, you bring a form of identification. This would preferably be a photo ID, but if it is not available other forms such as a bank card, or official letters addressed to you are acceptable.
Please arrive in good time for your appointment as this allows for the clinic to run smoothly. Your consultation will last around 30 minutes.

Following the consultation
In some cases, I can provide medical copies. When using medical records that are of direct relevance to any injury sustained at the time of your accident, they will be included in the report. All other aspects of your medical history will remain confidential.
I will use the information that you have provided along with the examination findings to prepare my report on your injuries for use about your claim.
